New Clients

The intake forms are below. Please print, complete, and bring your forms to your appointment.  If you are a distance client, you may e-mail all of your documents to christine@herbchick.com. Thank you for your cooperation.

There are 4 forms to complete and submit before to your appointment: 

1. Completed New Client Form (ADULT, CHILD or ANIMAL)

2. Signed Payment Policy

3. Signed Service Disclaimer 

4. Signed Energy Work Disclaimer  

5. Ion Foot Cleanse form is only needed when this appointment is requested.

6. A Food Log.  You can use the template below or make your own.

7. Please bring a dedicated notebook to ALL consultations.  Here you will record your programs, notes and observations for your sessions and any questions you may have for your follow up appointments. This will become a very valuable resource. 

8.  If you are currently taking medications, please speak with your pharmacist about any and all side effects, contraindications, physiological depletions and purpose given. 

9. Details are important.  Nothing is trivial. If it crosses your mind I want to know about it. Take your time. Look over your forms and put them down and revist and add to it the next day.  The more we have the better.

10. Don't be limited to boxes and lines.  IF you feel you need to add a page please do so.
